Context

Rwanda's rural district hospitals historically faced chronic blood-product stockouts, short-shelf-life wastage and long overland delivery times during obstetric and trauma emergencies, prompting the government to build a purpose-designed national regulatory pathway for autonomous drone flights rather than adapt existing aviation rules.

Objectives

The aim was to let Zipline's fixed-wing delivery drones fly beyond visual line of sight nationally, cutting delivery times and blood-unit wastage for rural hospitals as the network expanded from an initial hub in Muhanga to a second hub in Kayonza.

Activities

Starting in 2016, the government built the BVLOS regulatory sandbox and let Zipline operate distribution centres, first in Muhanga and then, from 2019, in Kayonza, delivering blood and medical products to rural hospitals.

Results

A retrospective time-series study in The Lancet Global Health covering 20 health facilities found drone deliveries averaged 49.6 minutes, 79 minutes faster than road delivery, and cut blood-unit wastage by 67% in the first year; a later study co-authored with Rwanda's Ministry of Health linked drone-served facilities to a 51% reduction in postpartum-hemorrhage deaths, a 30% reduction in trauma mortality, and a 63% fall in the blood inventory hospitals needed to hold.

Conclusions

The regulatory sandbox, originally built for one narrow health-logistics use case, has since become durable national infrastructure, with a December 2022 partnership targeting roughly 2 million deliveries and 200 million autonomous kilometres by 2029 and opening the network to other government agencies.
